The registration fee includes program materials, continental breakfast, lunch, welcome reception on October 9, dinner on October 10, and validated parking. Registrations will be accepted as long as space is available.

Manufacturers and distributors of pertinent equipment and pharmaceuticals will exhibit at this meeting. Time will be set aside to allow participants to visit the exhibits.
Hotel Accommodations:
A limited number of rooms are available at the JW Marriott Ihilani Resort & Spa at a special group rate of $259 per night plus tax, run of house accommodations, single or double occupancy, and subject to applicable state and local taxes (currently 11.962%) in effect at the time of check-in.
